What: RUSD EMPLOYEE HEALTH FAIR
When: 10/19/2010, 1:00 PM – 4:00 PM
Where: Nogales High - "The Manor"

All eligible employees must re-enroll int he the new CalPERS plan of your choice. If you do not complete the CalPERS enrollment form, you will not have health benefits on 1/1/2011.

*What should you bring to Open Enrollment?

Names of all dependents
Dates of birth of all dependents (new health reform law allows adult dependents up to age 26)
Social Security numbers of all dependents
Birth Certificate or adoption papers if you adding a new dependent
Marriage license, if you are adding a new spouse
domestic partner registration, if you are adding a new domestic partner

**What If I have coverage elsewhere and want a waiver?

If you are currently waiving medical coverage, you do not need to attend.
If you are currently covered but want to initiate a new waiver, please fill out form that was sent out.
